To notify us of a claim,
Call our Claims department (800) 233-3750 ext. 502 and have the following information ready:
- Name(s) of deceased
- Contract number(s)
- Date of death
- Cause of death
- Your relationship to deceased
or Complete a California Claim Notice C-8044 and submit it to us by mail, fax or email.
Once we are notified, we will send forms and instructions to file your claim.
